For a renewal of a State of DE CCDW permit (This is NOT for the Federal HR 218 permit)

Fill out the downloaded form and include the statutory filing fee of $65.00.

Cash, money order or personal check, made payable to “PROTHONOTARY” will be accepted.
Include Two passport photos and have the form notarized.

Mail to the Prothonotary Office in the County in which you are applying:
